Most people are told how to treat a burn. Very few are told what happens afterwards.
Once a burn has closed, the skin looks finished. It isn't. Underneath, the scar continues to remodel for anywhere from twelve to twenty-four months, and across that window it is still actively changing shape, colour, texture and flexibility. What you do during that period genuinely affects how the scar ends up.
This is the part of recovery that gets the least attention, and it's where the most difference can still be made. Below are direct answers to the questions people ask most about burn and radiation scars.
What actually helps a burn scar?
Consistent hydration and occlusion are the two interventions with the strongest evidence behind them for burn scars. A burn disrupts the skin's barrier, so scar tissue loses water far faster than healthy skin. Replacing that moisture and sealing it in improves itch, tightness and suppleness, and influences how the scar matures over the following months.
This is why hydration sits at the centre of international scar management guidance rather than at the edges of it. It is not a comfort measure. It is the intervention.
What separates a medical burn scar cream from an ordinary moisturiser is how long it holds that hydration in place. ALHYDRAN was formulated for exactly this, forming a semi-occlusive barrier that reduces trans-epidermal water loss from scar tissue.

Do burn scars go away completely?
Deeper burns almost always leave a permanent mark, but how that mark looks and feels can change substantially. Colour, height, flexibility and comfort all improve with consistent care during scar maturation. First-degree burns usually heal without scarring at all; second and third-degree burns carry a high risk of lasting scar tissue.
The realistic goal of scar care is a flatter, paler, more comfortable and more flexible scar, not the removal of the scar.
How long do burn scars take to fade?
Burn scars typically take twelve to twenty-four months to fully mature. Redness settles gradually across that period as the scar remodels. Because the tissue is still actively changing throughout, this window is when treatment has the greatest effect, and why scar care started early and continued consistently outperforms occasional use later.
Why is my burn scar so itchy?
Burn scars itch because the damaged skin barrier can no longer hold moisture, leaving scar tissue chronically dehydrated and irritated. The itch is often worst at night. Regular hydration is the first-line response and usually brings noticeable relief.
If itching is severe enough to disturb your sleep, see your doctor. Persistent scar itch has other treatment options and does not have to be endured.
When can I start putting cream on a burn scar?
Start once the wound has fully closed and the skin is dry, not before. Scar creams including ALHYDRAN are aftercare products and should never be applied to open wounds. Applying cream too early can interfere with healing.
Once you do start, apply twice daily and include the skin surrounding the scar, not only the scar tissue itself.
What is the best cream for burn scars in NZ?
The strongest burn scar creams are the ones with published clinical evidence in burn scars specifically, rather than general moisturisers or cosmetic scar oils. In New Zealand the main evidence-backed options are silicone gels and medical hydrating creams. ALHYDRAN is the medical hydrating cream supplied to New Zealand clinics, and it was developed in burn centres rather than adapted from general skincare.
That origin is the practical difference. ALHYDRAN combines freshly processed aloe vera with occlusive oils to reduce trans-epidermal water loss, and it has been tested on burn scars rather than on scars in general. A study at Ghent University published in the journal Burns compared silicone products against a hydrating gel-cream across three measures: how well each sealed the skin, how effectively it hydrated scar tissue, and how long the effect lasted. Controlled skin lesions allowed treated skin, untreated skin and healthy skin to be compared directly.

Further work has been presented to the European Burn Association over more than a decade, including two years of clinical experience from the Burn Centre at University Hospital Ghent, quality-of-life data from the Burn Rehabilitation Centre at Bad Klosterlausnitz, and research on water distribution in burn scars from OSCARE in Antwerp.

Are there before and after photos of burn scar treatment?
Yes. ALHYDRAN publishes documented clinical case studies with before and after photographs, covering hot water burns, flame burns, second degree burns and scars following skin grafts. Each case sets out the injury, the treatment timeline and the visible result, some tracked over a year or more.
They are worth looking at if you want a realistic sense of what scar maturation actually looks like over time, rather than a marketing image.

A word of warning: some of the earlier photographs show serious burn injuries and are confronting. If you are recovering from a burn yourself, you may prefer to look at these with someone else, or skip them entirely. Individual results vary, and the cases shown were treated under clinical supervision.
Can you treat old burn scars?
Yes. Established burn scars respond more slowly than scars still within the twelve to twenty-four month maturation window, but hydration still improves suppleness, itch and general comfort in older scar tissue. Colour change is less likely once a scar has fully matured.
What cream should I use during radiation treatment?
A cream for irradiated skin needs to be free of metals such as zinc, fragrance-free, and absorb readily so it can be applied without rubbing tender skin. ALHYDRAN meets all three requirements and is used in radiotherapy centres internationally, which is why it is stocked by New Zealand radiation oncology providers.
Metals can interfere with treatment, and fragrance is a common irritant on skin that has become highly reactive. Many general moisturisers and aqueous creams contain one or both, which is why they are often unsuitable during a course of radiation therapy.
Always check with your oncology team before adding any product. They know your treatment plan, and creams applied over radiotherapy skin markings can blur them.
What is radiation dermatitis?
Radiation dermatitis is the skin reaction that develops in the treatment field during a course of radiation therapy. Skin becomes progressively drier, redder and more irritated, and in more severe cases can blister or open. It is a common and expected side effect rather than a sign that something has gone wrong.
The underlying problem is the same one burn patients face: a disrupted skin barrier losing moisture faster than it can retain it. That is why the same care principles apply.
ALHYDRAN is used in burn units and radiotherapy centres internationally, and was developed specifically for skin in this condition.
Is ALHYDRAN used by clinics in New Zealand?
Yes. OneUp Healthcare supplies ALHYDRAN to New Zealand hospitals, clinics and radiation oncology providers, including Auckland Radiation Oncology.
Products supplied into clinical settings are held to a different standard than retail skincare. They have to be appropriate for compromised skin, compatible with active treatment, and backed by evidence a clinician is willing to stand behind.
Do burn scars need sun protection?
Yes. Burn and radiation-affected skin stays UV-sensitive for years, and sun exposure during scar maturation causes pigmentation changes that are difficult to reverse. Covering the scar or using a dedicated SPF product is important well beyond the point at which the skin looks healed.
In New Zealand this matters more than most people realise, given local UV levels. ALHYDRAN SPF30 combines the same hydration with broad-spectrum protection.
When should you see a doctor about a burn scar?
See a doctor if a scar is not healing, is growing beyond the original wound boundary, is restricting movement, shows signs of infection, or is causing pain or itch severe enough to affect sleep. Burns to the face, hands, feet or joints, and any burn larger than the palm of your hand, should always be medically assessed.
- Not healing, or reopening
- Growing beyond the original wound boundary, which may indicate a keloid
- Restricting movement across a joint
- Hot, swollen, weeping or otherwise showing signs of infection
- Causing pain or itching severe enough to affect your sleep
Scar care is not a substitute for medical assessment.
